Payment Plan A - Annual 18 Years*

• One Time Family Registration Fee ( $155 payable at time of registration)

• Sample Processing Fee ($795 payable at time of cord blood storage) and
  Annual Sample Storage Fee ( $125 payable at time of cord blood sample storage,
  representing first year of Sample Storage Fee).

• Thereafter,  Annual Sample Storage Fee ( $125 payable per year,
  on the birth month of the child, for 17 Years)

Payment Plan B - Annual 18 Years - 3 Payments*

• One Time Family Registration Fee ( $155 payable at time of registration)

• Three payments, representing the Sample Processing Fee and first year of
  Annual Sample Storage Fee:

  1st    $390 payable at time of cord blood sample storage

  2nd   $290 payable 3 months after the birth of the child

  3rd    $290 payable 6 months after the birth of the child

• Thereafter,  Annual Sample Storage Fee ( $125 payable per year,
  on the birth month of the child, for 17 Years)

Payment Plan C - Prepaid 18 Years*

• One Time Family Registration Fee ( $155 payable at time of registration)

• Sample Processing Fee($795 payable at time of cord blood sample storage ) and
  Prepaid Annual Sample Storage Fee ($1800 payable at time of cord blood sample storage,
  representing 18 years of Sample Storage Fee.)

• Thereafter, no Annual Sample Storage Fee due.

Payment Plan D - Prepaid 18 Years - 6 Payments*

• One Time Family Registration Fee ( $155 payable at time of registration)

• Six  payments, representing the Sample Processing Fee and Prepaid Annual
  Sample Storage Fee (18 years of Sample Storage Fee):

  1st              $395 payable at time of cord blood sample storage

  2nd – 6th    payable in 5 consecutive monthly instalments of $465 each,
  commencing 1 month after the birth of the child.

• Thereafter, no Annual Sample Storage Fee due.
