Lifebank Corp. is committed to upholding the confidentiality and security of personal information. Lifebank Corp. respects the rights of individuals to privacy and has instituted practices that will help ensure that personal information is handled responsibly. The Lifebank Privacy Policy is modeled after the Canadian Standards Association Model Code for the Protection of Personal Information, and is compliant with federal and provincial privacy laws including the federal Personal Information Protection and Electronic Documents Act (PIPEDA).

Personal Information:

The term “personal information” is used to describe most types of information that can be linked to an identifiable individual, e.g. name, home address, telephone number, social insurance number, medical and health records, and includes information about a client’s account, credit information and billing records.

Lifebank needs to collect, use, retain and disclose personal information to provide services to our clients and to manage our business. We will request only the personal information necessary to provide our services, and will inform individuals how we intend to use this information. To ensure that personal privacy is protected, Lifebank is committed to upholding the following ten principles of fair information practices:

1. Accountability:

Lifebank is responsible for the personal information under its control and has designated our Chief Executive Officer as our Privacy Officer to be responsible for addressing privacy concerns and ensuring Lifebank’s compliance with the procedures and principles set out in this document. This Privacy Officer has the responsibility of answering questions and concerns regarding the Lifebank Privacy Policy.

2. Identifying Purposes:

Lifebank identifies the purposes for which personal information is collected at or before the time the information is collected. The purposes for which information is collected, used or disclosed by Lifebank are those that a reasonable person would consider are appropriate under the circumstances or as otherwise required or permitted by law.

3. Consent:

We will obtain the consent of an individual prior to collecting, using or disclosing the individual's personal information, unless the law authorizes collection, use or disclosure without express consent. To enable an informed decision about the information provided, we will clearly explain the reasons for collecting personal information.

4. Limiting Collection:

Lifebank protects individual privacy by limiting the collection of personal information to that which is necessary for the purposes of providing our services and managing our business. Personal information will be collected by fair and lawful means.

5. Limiting Use, Disclosure and Retention:

Lifebank will not use or disclose personal information for purposes other than those consented to, or as required by law. Should Lifebank require the services of third parties to handle information, we will require them to adhere to privacy procedures that will keep information confidential. These third parties will be given only the information necessary for performing their information-handling services. Lifebank will retain personal information only as long as is necessary for the purposes for which it was collected. Records related to our cord blood collection and storage services will be retained for an appropriate period of time in conformance with the reference standards for Cord Blood Services.

Should Lifebank be required to delete or destroy records containing personal information, this will be done in ways that will ensure continued privacy.

6. Accuracy:

Lifebank is committed to ensuring that our records of personal information are accurate, complete, and up-to-date. Although it is the responsibility of our clients and other individuals from whom we obtain personal information to inform us of any relevant changes in personal information, we will take steps to ensure that our records are updated accordingly.

7. Safeguards:

Lifebank uses appropriate safeguards to protect the privacy of personal information. These safeguards are designed to prevent unauthorized access, disclosure, copying, use, or modification. Records are accessible only to authorized personnel who have been given the authority to access them and only as is necessary to provide services.

8. Openness:

So that you can be confident that Lifebank is handling your personal information appropriately, Lifebank will make information about its policies and practices concerning management of personal information readily available.

9. Individual Access:

If a client has, at any time, a question about our records containing personal information, we will do our best to answer it. A client has the right to be told about the kind of personal information Lifebank maintains, how it is used, and with whom it is shared.

Clients are entitled to challenge the factual accuracy as well as the completeness and relevance of our records. Incorrect information will be promptly corrected or, if necessary, deleted.

10. Addressing Comments and Concerns:

NOTICE TO CLIENTS

In order to provide our services to clients, it is necessary for our clients to provide certain personal information, such as their name, contact information, health and medical information, as well as credit card information. Lifebank will not, without a client's consent, collect, use or disclose any personal information, except to the extent necessary to provide services to the client or manage its businesses or as otherwise, required by law.

We do not sell personal information. In certain circumstances we may share personal information relating to a client with a hospital, laboratory, physician or other healthcare or service provider, in each case, only to the extent necessary to provide service to the client or as otherwise required by law.

Lifebank respects the privacy of visitors to its website. Personal information is collected, used, and disclosed by Lifebank only in accordance with this Privacy Policy.

Visitors to the Lifebank website have the option of revealing or not revealing any personal information. We collect personal information only when knowingly provided by a visitor, for example when the site registration form is completed or emailed to us for more information. Accessing Lifebank's website, indicates consent to our collection, use, and disclosure of the information provided.

1. Information Collection and Use

For information or services requested through Lifebank's website, personal information may be requested. For example, name, address, telephone number will be required by us when requesting a client information package. At that time the option to be added to our mailing list will be offered. We collect this information to enable us to send out our information packages. Lifebank may to use geographical information to generate statistics and reports for internal use.

2. Non-identifying Information

Lifebank logs certain technical information from an individual’s computer’s web browser when visiting us on the Internet. This information includes the Internet IP address, the address of the individual’s website, the amount of time the individual spends visiting our website, the pages visited, and web browser software used, and, if linked to us through a search engine, the keywords used to find our website. The logs do not record personal information, can be accessed only by Lifebank, and are used solely to track visits to our site and improve its content and layout. Lifebank never attempts to tie this information to particular individuals visiting our website, unless it is necessary for investigating security breach.

3. Disclosure and Retention of Information

The personal information provided through our site is never disclosed to third parties without consent. We DO NOT sell or trade personal information to other organizations or companies.

The account information provided when registering with us is stored in our secure databases. Lifebank must be contacted when an individual no longer wants information to be kept by us. When an individual contacts Lifebank with a question or comment, any identifying information is normally kept long enough for us to provide a response. If circumstances require us to keep it longer than that, the Individual will be notified.

4. Accuracy of Information

Visitors to Lifebank's website are responsible for the accuracy of information that they provide to Lifebank.
