Step-by-Step Guide to Registration

Before you start: Please make sure you have either downloaded our Information package or you have received it in the mail.
 

 STEP 1

Begin by reading the document entitled ‘Individual Account – Informed Consent’. The purpose of this document is to fully explain the reasons for storing your baby's cord blood and the procedures involved, as well as the financial aspects and limitations. Once you’ve read it thoroughly, complete and sign it.
 

 STEP 2

Next, refer to the document entitled ‘Individual Account – Client Service Agreement’. This document outlines the responsibilities of both Lifebank, and the Client (the expectant mother), in obtaining, shipping, and storing the cord blood sample following delivery. Once you’ve read it thoroughly, complete and sign it.
 

 STEP 3

Finally, refer to the document entitled ‘Client Information Health Questionnaire’. This document asks questions pertinent to your personal and family medical history. This information assists in determining suitability of your baby’s cord blood sample for long term storage.
 

 STEP 4

Once you’ve read and understand these three important documents, they must be completed, signed, and returned to Lifebank by fax or mail.
